Centene is seeking a dedicated Patient Care Advocate to support members and providers, close care gaps, and improve the overall healthcare experience. This role involves outreach, face-to-face member education, and collaboration with providers to enhance HEDIS measures, quality improvement, and compliance. Ideal candidates have a healthcare background, strong communication skills, and a passion for patient advocacy in managed care settings.



📋 Key Responsibilities
  • Act as a liaison between members, families, providers, and facilities.

  • Conduct face-to-face education with members about care gaps and preventive services.

  • Schedule doctor appointments and assist members with wraparound services such as transportation and community resources.

  • Perform telephonic outreach and health coaching to support quality improvement initiatives.

  • Collaborate with Provider Relations to improve provider performance in quality, risk adjustment, and operations.

  • Advise providers on HEDIS measures and coding standards (ICD-10 / CPT).

  • Document all member contacts and actions accurately.

  • Refer members to case or disease management as appropriate.

  • Complete special assignments and departmental projects.

🧩 Requirements
  • Bachelor’s degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related field; or equivalent work experience (total 4 years required).

  • 2+ years experience in direct patient care, social work, quality improvement, or health coaching, preferably in managed care.

  • Preferred licenses/certifications: LPN, LMSW, C-SW, LSW, or RN.

  • Strong communication, organizational, and problem-solving skills.

  • Ability to travel up to 75% within Indiana and work independently in remote or field settings.
