Devoted Medical is seeking a compassionate, mission-driven Nurse Practitioner (APRN) to join their next-generation virtual Responsive Care Clinic. This is a high-impact telehealth role designed for clinicians passionate about chronic disease management, senior care, and innovating how medications and results are managed for older adults across the country.
As part of a dynamic care team, you’ll support medication renewals, manage chronic conditions, assist with results management, and conduct virtual visits — all while helping expand access to essential care for at-risk seniors. This role is perfect for an NP with strong clinical judgment, empathy, and an entrepreneurial spirit.
📋 Key Responsibilities
Manage a high-volume medication refill queue for Devoted Medical’s primary care and “Medications on Time” service.
Conduct virtual visits for members requiring clinical evaluation before medication renewal.
Diagnose and manage chronic conditions such as hypertension, hyperlipidemia, and diabetes.
Assess risk factors, comorbidities, contraindications, and provide guidance on medication adherence.
Collaborate closely with PCPs, endocrinologists, cardiologists, pharmacists, nurses, social workers, caregivers, and family members.
Use and help improve Devoted’s home-grown electronic health information system.
Maintain accurate medical records that meet legal and ethical standards.
Participate in quality improvement initiatives and stay current on cardiometabolic best practices.
Ensure compliance with all telehealth, privacy, and regulatory requirements.
🧩 Preferred Qualifications
APRN with 2+ years experience in outpatient clinical practice (primary care, urgent care, cardiology, endocrinology, nephrology, etc.).
Strong understanding of chronic disease management, medication therapy, and lifestyle modification strategies.
Experience with telehealth and electronic health records (EHR).
Multistate licensure or willingness to obtain/maintain multiple state licenses.
Preferred: Proficiency in English and Spanish.
Knowledge of managed care, STARS/HEDIS measures, and closing care gaps is a plus.
Excellent clinical and non-clinical judgment, empathy, thoroughness, and adaptability.
