As a Care Coordinator, you will work closely with the Care Team Lead and the Member Services Manager to ensure the team has what it needs to support patients through the copay assistance process. You will respond to incoming member inquiries, place outbound calls, review claims, and maintain accurate patient data for ongoing follow-up.This role involves handling sensitive health information (PHI), completing time-sensitive tasks, and supporting the operational flow of patient enrollment. It’s an excellent match for pharmacy technicians or professionals with pharmacy/PBM experience who enjoy service-focused, detail-heavy work from home.
📋 Key Responsibilities
Provide responsive support to patients and internal teams as needed.
Schedule and coordinate member implementation calls with the Care Team Lead.
Conduct ad hoc claims reviews when required.
Identify active patients, review utilization history, and determine next eligible coverage dates.
Assist patients with enrolling in manufacturer-sponsored copay assistance programs.
Maintain accurate patient databases for tracking, reporting, and follow-up.
Review daily prior authorization updates, rejected claims, and paid claim notifications.
Handle duties requiring access to Electronic Protected Health Information (PHI).
Adapt to workflow changes, shifting priorities, and unexpected needs.
Perform additional duties as assigned.
🧩 Requirements
Required:
Certified Pharmacy Technician (CPhT) OR 2–5 years of experience in a retail pharmacy or pharmacy benefit management setting.
Excellent written and verbal communication skills.
Proficiency with Microsoft Office Suite.
Professional and confident phone presence.
Ability to maintain confidentiality and handle sensitive health information appropriately.
