Imagine Pediatrics is hiring remote LPNs to support children with complex medical needs after emergency department visits — helping families navigate follow-up care, avoid unnecessary readmissions, and feel supported every step of the way.

💬 Job Overview

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group focused on caring for children with complex medical needs. As a Licensed Practical Nurse supporting Emergency Department Follow-Up, you’ll work remotely to assist families after ED visits — helping coordinate care, identify concerns early, and ensure children receive the follow-up support they need to recover safely at home.


This role blends clinical knowledge with care coordination, education, and advocacy in a collaborative virtual environment.



📋 Key Responsibilities
  • Proactively engage patients and families following emergency department visits.

  • Provide telephonic and virtual follow-up visits focused on discharge support and care coordination.

  • Educate families on emergency department avoidance, resources, and Imagine Pediatrics services.

  • Identify symptom worsening or concerns and escalate appropriately to the clinical team.

  • Support care coordination needs such as pharmacy fills, PCP follow-ups, and social determinants of health (SDOH).

  • Ensure patients have appropriate follow-up appointments scheduled.

  • Schedule callbacks with Imagine Pediatrics clinical teams as needed.

  • Coordinate with hospital discharge planners and Imagine Pediatrics Transition of Care Nurses.

  • Participate in daily team huddles to discuss patient progress and needs.

  • Provide referrals to internal services, social services, and community-based resources.

  • Serve as a liaison with hospitals and health systems to promote awareness of Imagine Pediatrics’ services.

🧩 Requirements
  • Licensed Practical Nurse (LPN) — active license required.

  • 2+ years of relevant clinical experience.

  • Experience in emergency department or inpatient settings preferred.

  • Experience with medically complex pediatric patients preferred.

  • Knowledge of Medicaid populations preferred.

  • Bilingual Spanish required.

  • Associate’s degree or higher preferred.

  • Ability to work independently in a remote, multi-platform environment.
