💬 Job Overview

CareCentrix is hiring Patient Advocates with Medical Assistant experience to support patients recently discharged from the hospital. In this role, you’ll work in a high-volume call center environment making outbound calls and supporting inbound queues to assess non-clinical needs, coordinate services, and ensure patients are recovering safely at home.


This is a non-clinical role focused on patient outreach, care coordination, documentation, and communication with clinical teams — ideal for Medical Assistants who enjoy patient interaction and structured workflows in a remote setting.



📋 Key Responsibilities
  • Conduct outbound outreach to recently discharged patients.

  • Answer inbound patient calls from a queue.

  • Ask non-clinical screening questions to identify post-discharge needs.

  • Determine eligibility for readmission prevention programs.

  • Coordinate services such as transportation and appointment scheduling.

  • Document patient data accurately to support clinical program goals.

  • Escalate concerns to appropriate clinical or specialty teams.

  • Communicate with physicians and clinical staff as needed.

  • Participate in performance improvement and quality initiatives.


🕒 Training & Schedule
Training & Nesting Period: 4–6 weeks
  • Training Hours: Monday–Friday, 8:30 AM – 5:30 PM EST

  • Post-Training Schedule:Monday–Friday, 9:00 AM – 6:00 PM EST

  • ⚠️ Evening or weekend coverage may be required based on business needs.


🧩 Requirements
  • High school diploma or GED required.

  • 1+ year experience as a Medical Assistant (required).

  • 1+ year customer service experience in a call center preferred.

  • Strong empathy and ability to build rapport over the phone.

  • Comfort working in a high-volume call center environment.

  • Intermediate keyboarding skills (35+ WPM while active listening).

  • Ability to navigate dual monitors and multiple applications.

  • Basic PC and Microsoft Office proficiency.

  • Strong attention to detail and HIPAA compliance.
